How to Find Your Test Kit ID
Your Test Kit ID is a unique identifier printed on a barcode label that links your sample to your account. Follow the instructions below for your specific test kit type.
Omega-3 & Vitamin D Test Kit
Your Kit ID is located in here:
Location: Sample Collection Card (Inside the sealed plastic bag)
- Open your test kit and locate the sealed plastic bag
- Inside, you'll find a white card labeled "Sample Collection Card"
- Look for the barcode label on the front of the card
- Below the barcode, you'll see your Kit ID starting with "USA" followed by numbers
ID Format: Starts with USA followed by letters or digits
Example: USAF123456
NAD+ Test Kit
Your Kit ID is located in TWO places:
Location 1: Sample Collection Card (Primary location)
- Open the sealed plastic bag inside your test kit
- Find the white "Sample Collection Card"
- Look for the barcode label on the card
- Your Kit ID is printed below the barcode with an asterisk (*) at the start and the end
Location 2: Outside of Box
- Check the side edge of the outer box
- Locate the barcode sticker
- The same Kit ID appears below the barcode
ID Format: Starts with DNA followed by letters and numbers
Example: DNAXYZ123S
⚠ Important:
- DO NOT Include the asterisk (*) when entering your Kit ID
- This ID is case-sensitive
- The ID may contain both letters and numbers after "DNA"
BioAge Test Kit
Your Kit ID is located in TWO places:
Location 1: Collection Tube Label (Primary location)
- Open your orange BioAge test kit box
- Locate the white collection tube with white cap
- Find the barcode sticker wrapped around the tube
- Your Kit ID is printed below the barcode starting with "M"
Location 2: Outside of Box (On the side edge)
- Check the white side edge of the orange box
- Look for the barcode label on the narrow side panel
- The same Kit ID is printed below this barcode
ID Format: Starts with M followed by numbers and letters
Example: M0000A12B3
⚠ Important:
- Use the ID from the collection tube label
- The letter "M" is always uppercase
- Include both numbers and letters exactly as shown
- This ID is case-sensitive
Can't Find Your Kit ID?
Common Issues:
- Multiple barcodes on packaging: Use the barcode on your sample collection card or tube, NOT shipping barcodes
- Barcode difficult to read: Try different lighting angles or use your phone camera to zoom in
- Plastic bag still sealed: You need to open the sealed bag to access the collection card inside
- Already disposed of packaging: Check the sample collection card or tube - the ID should still be there
Which ID Should I Use?
Always use the ID from:
- Omega-3 / Vitamin D: Sample Collection Card (inside sealed bag) - starts with USAF
- NAD+: Sample Collection Card (inside sealed bag) - starts with DNA
- BioAge: Collection Tube label - starts with M
